Registration Fee $250.00 – we encourage all families to pay via paypal online as this will allow for quicker checkout at registration:

Links to the registration forms can be found below. These forms are now in fillable format. Please complete, print out and bring to registration along with the following items:

1. Player Contract: 2012_PW_Player_Contract
2. Player Physical Form: 2012_PW_Physical_Fitness_And_Medical_History_Form
3. Copy of Birth Certificate
4. Current Utility Bill for Proof of Residence
5. Payment $250.00 or if paid via paypal your receipt as proof of payment
6. 2×3 Picture of participant

Items you will need to provide at a later date in order to start practice:

1. Copy of your child’s final report card 2011-2012 school year – must show 4th quarter with final grades
2. If you did not provide the physical form signed by a medical professional at registration this must be turned in prior to the first day of practice. This must be dated anytime after 1/1/12 and must be completed on the attached Pop Warner physical form.
